Schedule:

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, ongoing conditions, and medication regimens
• Offer patient education on disease management and medication adherence
• Identify gaps in clinical care, documenting and providing recommendations to support member health
• Focus on assessment and educationprescribing and ongoing treatment planning is not required

Qualifications:

• Active Nurse Practitioner License in Missouri
• Board Certification through AANP or ANCC
• Current BLS/CPR certification
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience preferred
