This per diem opportunity offers the flexibility to create your own schedule, working with Medicare Advantage plan members in various communities, including Brinkley, Clarendon, and Holly Grove. Assessments typically last about 45 minutes and may involve annual health evaluations or post-discharge visits, depending on patient needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own workdays and hours
- Minimum weekly or bi-weekly hourly commitments required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage health plan members
- Review current and past medical histories, chronic conditions, and medication regimens
- Educate patients about their medical conditions and medications to promote understanding and adherence
- Identify gaps in care and provide actionable recommendations
- No treatment plans or prescription of medications required
Qualifications:
- Active Nurse Practitioner License in AR
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of recent nurse practitioner experience preferred
- MSN degree from an accredited nursing program
