This per diem opportunity is designed for a Nurse Practitioner passionate about delivering high-quality care directly to individuals in the comfort of their homes. Youll perform comprehensive home health risk assessments for Medicare Advantage members throughout Hamilton County, including areas such as Cincinnati, Forest Park, and Norwood.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Commit to a minimum number of hours weekly or bi-weekly based on your availability
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, chronic conditions, and medication regimen
- Deliver patient education on medical conditions and medication administration
- Identify gaps in care, offering clear, actionable recommendations for patients or providers
- Focus solely on assessment; no treatment planning or prescribing medications required
- Each visit typically lasts approximately 45 minutes
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current experience as a Nurse Practitioner preferred
