An exciting per diem opportunity is available for a skilled Nurse Practitioner to provide crucial home health risk assessments throughout Onondaga County, NY, including the Syracuse area and surroundings such as Salina, Onondaga, and Clay. Schedule/Hours:

• Flexible shifts available between Monday and Sunday
• Freedom to design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage members
• Evaluate comprehensive medical history, current conditions, and medication usage
• Provide clear education to patients about diagnoses and prescriptions
• Identify healthcare gaps and suggest actionable recommendations for their resolution
• Perform post-discharge visits as needed
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in NY
• Board Certified (AANP or ANCC)
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum 6 months experience as an NP is preferred

This position offers a compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and administrative time payment, ensuring your effort and travel are always recognized.
