Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual risk assessments for Medicare Advantage plan members in their homes
  • Evaluate prior and current medical histories, health conditions, and medication histories
  • Educate patients and families on medical conditions and proper medication usage
  • Identify gaps in care and offer recommendations to support patient well-being
  • No responsibilities for treatment planning or medication prescribing required

Qualifications:

  • Valid NY State Nurse Practitioner license
  • Current CPR certification (BLS)
  • Board Certification (AANP or ANCC)
  • At least 6 months of current nurse practitioner experience is preferred
  • MSN degree from an accredited school of nursing
