Advance your clinical expertise as a Nurse Practitioner specializing in home health risk assessments across Oklahoma County. Enjoy the autonomy and flexibility of per diem scheduling, allowing you to tailor shifts to your needs. In this role, you will conduct comprehensive home health risk evaluations for Medicare Advantage members throughout Oklahoma City, Edmond, and Midwest City, supporting the health outcomes of our senior population.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments in patient homes, dedicating approximately 45 minutes per evaluation
- Review medical histories, current health status, medication usage, and identify any health gaps or risks
- Educate patients on medical conditions, prescriptions, and proactive health management
- Document findings, offering recommendations to improve care outcomes and close gaps in health services
- No medication prescribing or treatment plan creation required, focusing your skills on assessment and education
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current BLS/CPR certification
- Board Certification as an NP via AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN from an accredited nursing school
This position offers highly competitive compensation of $80$90 per completed evaluation, along with mileage reimbursement and compensation for administrative time.
