Deliver impactful care while enjoying flexible scheduling as a Nurse Practitioner providing home health risk assessments throughout Jackson County, FL. This per diem opportunity empowers you to create your own shifts, selecting from flexible options offered Monday through Sunday, with only a minimum weekly or bi-weekly hourly commitment.
Schedule:
- Flexible shifts offered seven days a week
- Create your preferred schedule

Primary Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Evaluate medical history, existing conditions, and medication regimen
- Educate patients regarding their health conditions and medications
- Identify gaps in care, offering recommendations to improve overall health management
- All visits are assessment-based: no treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner License in Florida
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited school of nursing
- Minimum of six months recent experience as a Nurse Practitioner preferred
Each home health assessment typically lasts around 45 minutes and may include annual health check-ups or post-discharge visits at the members residence. Assignments may occur in Marianna, Graceville, Sneads, or nearby areas within Jackson County. Compensation includes $80-$90 per completed evaluation, plus mileage reimbursement and compensation for administrative time.
