Exciting opportunity available for a skilled Nurse Practitioner to deliver compassionate care through home health risk assessments for Medicare Advantage plan members across Jackson County, FL. This per diem position provides extraordinary flexibilitychoose when you work, with shifts available any day of the week and the ability to create your own schedule.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduled, allowing ultimate flexibility
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage members in diverse communities throughout Jackson County, including Marianna, Graceville, and Sneads
- Review and evaluate medical histories, current medical conditions, and medication records
- Educate patients on their health conditions and appropriate medication use
- Proactively identify gaps in care and provide actionable recommendations without the need to formulate treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- At least six months of recent nurse practitioner experience is preferred

In return for your expertise, you will receive competitive compensation of $80$90 per evaluation, plus a mileage stipend and remuneration for administrative time worked.
