Make a meaningful impact on community health as a Nurse Practitioner specializing in home health risk assessments. This per diem role allows you to set your own schedule, working with Medicare Advantage plan members across Chincoteague Island and Accomack County. Each home visit averages 45 minutes, focusing on comprehensive annual health assessments or post-discharge evaluationswithout the need to prescribe medications or develop treatment plans. Flexibility, professional autonomy, and community engagement converge in this rewarding opportunity.
Schedule:
- Flexible shifts available from Monday through Sunday
- Self-schedulingdesign your own workweek
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage members
- Gather and review prior/current medical and medication history
- Educate patients on medical conditions and medication management
- Highlight gaps in care and recommend actions for improvement
- Documentation and follow-up as needed for compliance and reporting
- Travel within Chincoteague, Onancock, Parksley, and greater Accomack County
Qualifications:
- Unencumbered Nurse Practitioner license in Virginia
- Board Certification (AANP or ANCC)
- Masters degree in Nursing (MSN) from an accredited institution
- Current Basic Life Support (BLS/CPR) certification
- Six months or more of Nurse Practitioner experience preferred
This position offers $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
