Job Description

Elevate patient care in Rio Arriba County by providing in-home risk assessments as a Nurse Practitioner specializing in home health. In this per diem position, youll have the autonomy to create your own schedule with flexible shifts available between Monday and Sunday, meeting a minimum weekly or bi-weekly hourly commitment. Assessments are performed in patients homes across Espanola, Chama, Abiqui, and surrounding areas.

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Evaluate patients prior and current medical history, medical conditions, and medication use
  • Educate patients on their medical conditions, prescribed therapies, and effective medication management
  • Detect gaps in care and make informed recommendations for follow-up or additional resources
  • No requirement for prescribing medications or developing treatment plans

Schedule:

  • Flexible shifts available Monday through Sunday

  • Commitment to minimum weekly or bi-weekly hours

Qualifications:

  • Active Nurse Practitioner license in New Mexico
  • MSN degree from an accredited nursing program
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC required
  • At least 6 months of experience as a Nurse Practitioner preferred
