Schedule:

• Flexible shifts available Monday through Sunday

Key Responsibilities:

• Conduct annual home health risk assessments, which typically last 45 minutes per visit
• Evaluate patients past and current medical histories, conditions, and medication regimens
• Deliver education on disease processes and proper medication usage
• Identify any gaps in care; provide clear recommendations to address those issues
• Visits include annual assessments and post-discharge evaluations
• No requirement for treatment plan development or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Massachusetts
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• MSN from an accredited school of nursing
• Six months or more of current nurse practitioner experience preferred

This position offers a competitive stipend of $80-$90 for each evaluation, plus a mileage stipend and compensation for administrative time.
