Step into a dynamic per diem role tailored for experienced Nurse Practitioners passionate about impacting patient outcomes in the home health setting. This opportunity focuses on home health risk assessments for Medicare Advantage plan members throughout Hampden County, Massachusetts, including Springfield, Chicopee, and Westfield. Each assessment takes approximately 45 minutes, and you may be scheduled in any of these locations during your shift, ensuring a varied and engaging workday.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual in-home health risk assessments on Medicare Advantage enrollees
- Review and document comprehensive medical and medication histories
- Deliver education to members about their conditions and prescribed medications
- Detect gaps in care and suggest actionable recommendations to improve patient wellness
- Note: No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- MSN degree obtained from an accredited institution
- National board certification via AANP or ANCC
- Valid CPR certification (BLS)
- At least six months of recent Nurse Practitioner experience preferred
This per diem position provides highly competitive compensation, including $80$90 for each evaluation, as well as a mileage stipend and pay for administrative time.
