Job Description
As a Nurse Practitioner specializing in Home Health, you'll deliver vital risk assessments for Medicare Advantage plan members throughout Hampden County, including areas such as Springfield, Chicopee, and Westfield. This per diem opportunity offers the flexibility to tailor your schedule to fit your needs, with assignments available any day of the week. During each shift, you may be scheduled to complete assessments in different locations within the region.
Schedule:
- Flexible shifts available from Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive home health risk assessments, averaging 45 minutes, for Medicare Advantage members
- Review and evaluate medical and medication histories, as well as current medical conditions
- Provide clear education regarding diagnoses, health risks, and prescribed medications
- Identify care gaps, making recommendations to ensure members achieve optimal health outcomes
- No requirement to create treatment plans or prescribe medications; focus remains on assessment and education
Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Board Certified through AANP or ANCC
- MSN degree from an accredited school of nursing
- Current BLS/CPR certification
- At least six months of current experience as a Nurse Practitioner (preferred)
