A dynamic opportunity is available for a skilled Nurse Practitioner with experience in home health assessments. This per diem role requires conducting home health risk assessments for Medicare Advantage members throughout Beckham County, including areas such as Elk City, Sayre, and Erick. Schedule:

  • Flexible shifts available Monday through Sunday
  • Personalize your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments with Medicare Advantage plan members in the community
  • Assess patients prior and current medical histories, medical conditions, and medication usage
  • Educate patients regarding their medical conditions and prescribed medications
  • Identify potential care gaps and provide actionable recommendations for improvement
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Current and valid Nurse Practitioner license in Oklahoma
  • BLS certification (CPR)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of experience as a practicing NP preferred

You will benefit from compensation of $80-$90 per evaluation, as well as a mileage stipend and reimbursement for administrative time related to your visits. Each risk assessment averages 45 minutes, allowing you to efficiently support patients while managing your own workload.
