This per diem opportunity is ideal for a Nurse Practitioner with experience in home health, seeking the flexibility to design their own work schedule. Responsibilities center on visiting Medicare Advantage plan members throughout Chester County, conducting thorough reviews of their health status, and empowering patients with education and supportwithout the need to develop treatment plans or prescribe medication.

Schedule:

• Flexible shifts, Monday through Sunday
• Self-schedulingcreate your own hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-person annual health risk assessments in patient homes
• Assess medical history, existing conditions, and current medication regimens
• Offer clear education on medications and chronic health conditions
• Identify care gaps and make practical recommendations to improve patient outcomes
• Complete thorough documentation for each assessment

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• Masters degree in Nursing (MSN) from an accredited program
• At least 6 months of clinical experience as an NP preferred

This role offers a highly competitive rate of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
