Job Description
Deliver specialized home health care as a Nurse Practitioner conducting risk assessments for Medicare Advantage plan members. Utilize your expertise in patient evaluation and health education while enjoying the flexibility of a per diem, self-directed schedule.
Schedule:
- Flexible shifts offered Monday through Sunday
- Design your own work schedule
- Weekly or bi-weekly minimum hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments, typically taking 45 minutes per visit
- Review and document patients medical history, existing conditions, and medication use
- Educate patients about their medical diagnoses and prescribed medications
- Identify gaps in care and recommend appropriate follow-up actions
- No requirement to develop treatment plans or prescribe medications
Required Qualifications:
- Licensed Nurse Practitioner with valid credentials in VA
- Board certification through AANP or ANCC
- Current CPR/BLS certification
- MSN degree from an accredited nursing program
- Minimum of six months' current NP experience preferred
