Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage members throughout Camden County, NJ, including Cherry Hill and nearby areas. Schedule:

• Flexible shifts available Monday through Sunday

• Minimal weekly or bi-weekly hourly commitments required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members in their homes
• Evaluate patients current and previous medical histories, conditions, and medication regimens
• Offer education to patients on medical issues and proper medication use
• Identify care gaps, offering recommendations to address any discovered needs
• Assessment visits may include annual health reviews or post-discharge check-ins
• No responsibility for creating treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Current CPR/BLS certification
• Board Certification by AANP or ANCC
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited program
