Exciting per diem opportunity available for a skilled Nurse Practitioner with home health experience to conduct risk assessments for Medicare Advantage members. This role offers you the flexibility to design your own schedule with shifts available seven days a week. Locations throughout Orangeburg County, South Carolinaincluding areas such as Orangeburg, Holly Hill, and Northprovide a dynamic and rewarding setting for experienced practitioners.

Schedule:

• Flexible shifts offered Monday through Sunday
• Decide your own working schedule
• Minimum weekly or bi-weekly hours to meet your needs

Key Responsibilities:

• Complete annual and post-discharge home health risk assessments, each typically lasting about 45 minutes
• Review and evaluate medical history, chronic and current conditions, as well as medication use
• Educate patients about their health conditions and appropriate medication management during each assessment
• Identify gaps in care and communicate actionable recommendations
• No requirement to prescribe medications or formulate new treatment plans

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• Minimum of 6 months recent Nurse Practitioner experience (preferred)
• MSN degree earned from an accredited nursing program
