Job Description

Enjoy the autonomy and variety of providing expert care in a home health setting by performing risk assessments for Medicare Advantage plan members in Craig County, OK. As a trusted Nurse Practitioner, youll have a lasting impact, helping patients through annual health evaluations or post-discharge assessments in communities such as Vinita, Ketchum, Welch, White Oak, and nearby areas.

Key Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage plan members
  • Review medical history, ongoing health concerns, and current medications
  • Educate patients on their health conditions, diagnoses, and prescription regimens
  • Detect gaps in care and recommend practical solutions
  • Focus solely on assessmentno requirements to prescribe or develop treatment plans

Qualifications:

  • Active Nurse Practitioner License in Oklahoma
  • MSN degree from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of 6 months of experience as an NP is preferred

The role provides a competitive per-evaluation compensation of $80-$90, with additional mileage reimbursement and administrative time included.
