Explore a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments throughout Muskogee County. This position allows for flexible scheduling, making it ideal for practitioners seeking autonomy and balance. Assessments will be conducted in members homes, primarily serving individuals enrolled in Medicare Advantage plans. Each assessment is expected to take about 45 minutes and could range from annual wellness evaluations to post-discharge follow-ups. Locations may include Muskogee, Fort Gibson, Warner, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly commitment based on your availability
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate medical histories, including current conditions and medications
- Educate members about their diagnoses and treatment regimens
- Identify care gaps and recommend next steps to improve patient outcomes
- No requirements for creating ongoing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Oklahoma
- Board Certification (AANP or ANCC) required
- MSN degree from an accredited nursing program
- Current BLS/CPR certification
- At least six months of recent nurse practitioner experience preferred
