Job Description

Schedule / Hours

  • Flexible shifts available Monday through Sunday
  • Self-schedulingcreate your own work hours
  • Set your preferred weekly or bi-weekly hourly commitment

Responsibilities

  • Perform thorough annual health risk assessments for Medicare Advantage plan members, with each visit taking approximately 45 minutes
  • Review patients medical histories, current health conditions, and medication regimens
  • Educate patients about their health conditions and their medications, promoting ongoing wellness
  • Identify and report care gaps, making recommendations to close those gaps
  • Focus on patient evaluation without prescribing medication or developing treatment plans

Qualifications

  • Current Nurse Practitioner license in OK
  • Active CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • At least 6 months of recent NP experience is strongly preferred
  • MSN degree from an accredited nursing school
