Job Description
Take on a rewarding opportunity as a Nurse Practitioner delivering impactful care in a home health setting. This per diem role is focused on performing home health risk assessments for Medicare Advantage plan members throughout Hamilton County, TN, including areas in and around Chattanooga, East Ridge, and Soddy-Daisy.
Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own work schedule
- Minimum weekly or bi-weekly hourly commitment as determined by your availability
Responsibilities:
- Complete comprehensive annual home health risk assessments in patient homes
- Evaluate patients past and current medical histories, ongoing medical conditions, and medication regimens
- Educate patients regarding their health conditions and medication usage
- Identify gaps in care and provide detailed recommendations for follow-up actions
- Conduct both annual health and post-discharge assessment visits as assigned
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in Tennessee
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN from an accredited school of nursing
