Job Description
Deliver meaningful impact to patients in your community by conducting home health risk assessments for Medicare Advantage members in Gadsden County. As an experienced Nurse Practitioner specializing in Home Health, you'll play a valuable role in improving patient outcomes through preventive evaluation and educational support.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hour commitment required
Key Responsibilities:
- Perform comprehensive annual health risk assessments during in-home visits (average visit: 45 minutes)
- Review and evaluate prior/current medical histories, chronic conditions, and medication use
- Educate patients on disease management, medications, and health maintenance
- Identify care gaps and recommend solutions or follow-up, without the responsibility of prescribing or creating long-term treatment plans
- Complete detailed documentation during or immediately after the visit to ensure accurate records
Qualifications:
- Active Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Board Certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of six months recent NP experience preferred
- Strong communication skills and comfort with independently visiting patients in their homes
You will receive competitive compensation of $80-$90 per completed evaluation, with an added mileage stipend and administrative pay for required paperwork and travel.
