This role allows you to make a meaningful impact on Medicare Advantage plan members by traveling to clients homes within the greater Charlottesville and Albemarle County region, including Scottsville, Crozet, and Hollymead. Youll conduct thorough annual assessments or post-discharge visits, gathering essential health information and supporting patients ongoing care.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Meet a minimum weekly or bi-weekly hour commitment

Key Responsibilities:

• Complete in-home annual or post-discharge health risk assessments for region-based Medicare Advantage plan members
• Review each patients full medical and medication history
• Educate individuals about their health conditions and medication regimens
• Spot gaps in care, offering actionable guidance to bridge those gaps
• No need to develop treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner license (VA)
• Board certification from AANP or ANCC
• Current BLS/CPR certification
• Minimum six months current NP experience preferred
• MSN degree from an accredited nursing school
