Advance your career in home health by performing comprehensive risk assessments for Medicare Advantage plan members. This per diem opportunity empowers you to create your own schedule and work across diverse communities within Cabarrus County, ensuring every shift is both meaningful and rewarding.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan participants
- Assess patients medical history, current conditions, and medication regimens
- Deliver patient education regarding medical conditions and medications to foster better health management
- Recognize and address gaps in care, offering actionable recommendations for improvement
- No responsibility for writing prescriptions or developing detailed treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Master of Science in Nursing (MSN) from an accredited program
- Board Certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Minimum of six months recent Nurse Practitioner experience preferred
