Job Description

Step into an impactful per diem opportunity providing home health risk assessments as a Nurse Practitioner. This flexible position offers you the autonomy to create your own schedule, with shifts available throughout the weekincluding weekends. Primary responsibilities include performing comprehensive, in-home risk assessments for Medicare Advantage plan members residing within Wood County, covering communities such as Parkersburg, Vienna, and Williamstown.

Schedule/Hours:

  • Flexible shifts offered Monday through Sunday
  • Design your own schedule to suit personal and professional needs
  • Set preferred minimum weekly or bi-weekly hourly commitments

Responsibilities:

  • Conduct annual health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate prior/current medical history, medical conditions, and medication usage
  • Deliver patient education regarding diagnosed conditions and prescribed medications
  • Identify gaps in care, offering actionable recommendations for follow-up or additional support
  • Focus solely on assessment and educationno treatment planning or medication prescribing required

Qualifications:

  • Valid Nurse Practitioner license in West Virginia
  • Board Certification from AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience (preferred)
