Job Description

Deliver high-impact care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers the flexibility to create your own schedule and serve a diverse group of patients in Charleston and nearby communities within Mercer County, WV, such as South Charleston and St. Albans. Your expertise will make a difference as you evaluate, educate, and enhance patient outcomes while enjoying autonomy and variety in your daily work.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own daily or weekly work routine
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual health risk assessments in patients homes
  • Review prior and current medical history, including health conditions and medications
  • Offer education to members regarding medical issues and prescription regimens
  • Identify and address any gaps in patient care, providing recommendations as needed
  • No requirement to prescribe medication or develop treatment plans

Qualifications:

  • Current Nurse Practitioner (NP) License in West Virginia
  • Active CPR certification (BLS required)
  • Board Certified through AANP or ANCC
  • Preferred: A minimum of 6 months of recent NP experience
  • Master of Science in Nursing (MSN) from an accredited nursing program
