Advance your Nurse Practitioner career by providing vital home health services in Bedford County, TN. This per diem position centers on conducting in-home risk assessments for Medicare Advantage plan members located throughout Shelbyville, Wartrace, Bell Buckle, and surrounding areas. Your day-to-day will provide flexibility, autonomy, and the opportunity to make a direct impact on individuals health and wellbeing in their own homes.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment, tailored to your availability
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review patients medical history, current conditions, and medication regimen
- Offer education regarding medical conditions and medications during each assessment
- Identify care gaps and recommend solutions for improved health outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current Basic Life Support (BLS/CPR) certification
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
