This position specializes in conducting in-home health risk assessments for Medicare Advantage plan members, supporting preventative care, care coordination, and patient education. Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Customize your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members in their residences
- Evaluate patients medical history, current conditions, and medication regimen
- Deliver clear patient education regarding medical conditions, medications, and overall health management
- Identify care gaps and offer evidence-based recommendations for improvement
- No requirement to develop treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS certification (CPR)
- Board Certified through AANP or ANCC
- Minimum 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
