Job Description
Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments. This position provides the unique opportunity to work per diem in various parts of Durham County, including Raleigh, Morrisville, and Cary. Conduct in-home visits with Medicare Advantage members, playing a vital role in promoting preventative health and supporting positive patient outcomes.

Key Responsibilities:
- Conduct annual risk assessments and post-discharge visits for Medicare Advantage members in their homes
- Review both current and prior medical histories, chronic conditions, and medication lists with patients
- Educate patients on their health status and medications, supporting better understanding and adherence
- Identify care gaps and make recommendations to close those gaps
- Documentation onlyno prescribing or treatment planning required during visits
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification as a Nurse Practitioner (AANP or ANCC)
- Minimum of six months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
