Key Responsibilities:
- Perform comprehensive annual risk assessments in patients homes within the area, including Chandler, Stroud, Prague, and surrounding communities
- Review and assess previous and current medical history, medical conditions, and medications
- Educate patients about their health status and prescribed treatment regimens
- Identify gaps in patient care and offer practical recommendations for improvement
- Focus exclusively on assessmentsno treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current BLS (CPR) certification
- Board certification by AANP or ANCC
- At least 6 months of recent NP experience (preferred)
- MSN from an accredited School of Nursing
