This per diem opportunity is ideally suited for a Nurse Practitioner with home health expertise, looking to make a positive impact through comprehensive risk assessments. Serve Medicare Advantage plan members across Vanderburgh County, including Evansville and surrounding communities, with each evaluation averaging 45 minutes. Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling and autonomy to tailor your weekly or bi-weekly shifts
Core Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review medical history, current conditions, and medication regimens
- Educate patients about their diagnoses and prescriptions to ensure full understanding and compliance
- Identify care gaps, providing evidence-based recommendations to improve patient outcomes
- Focus exclusively on assessmentsno medication prescribing or treatment planning required
Qualifications:
- Valid Nurse Practitioner license in Indiana (IN)
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
