Serve Medicare Advantage plan members across Vanderburgh County, including Evansville and surrounding communities, with each evaluation averaging 45 minutes. Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling and autonomy to tailor your weekly or bi-weekly shifts

Core Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review medical history, current conditions, and medication regimens
  • Educate patients about their diagnoses and prescriptions to ensure full understanding and compliance
  • Identify care gaps, providing evidence-based recommendations to improve patient outcomes
  • Focus exclusively on assessmentsno medication prescribing or treatment planning required

Qualifications:

  • Valid Nurse Practitioner license in Indiana (IN)
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience preferred
