Take on a pivotal position as a Nurse Practitioner providing home health assessments throughout Cape Girardeau County and surrounding areas. This per diem role offers schedule flexibility and the opportunity to deliver high-impact care directly to Medicare Advantage plan members in their homes. Youll connect with patients in Jackson, Dutchtown, Millersville, and similar community settings while maintaining professional autonomy.

Schedule:

• Flexible shifts: Monday through Sunday
• Create your own schedule based on availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual health risk assessments for Medicare Advantage members in home settings
• Evaluate patient medical history, current conditions, and medication regimen
• Educate patients on diagnoses, treatments, and medication usage
• Identify care gaps and recommend strategies for resolution
• Complete assessments, but treatment planning and medication prescribing are not required

Qualifications:

• Active Nurse Practitioner license in Missouri (MO)
• Current CPR certification (BLS required)
• Board certification via AANP or ANCC
• At least 6 months of current NP experience preferred
• Master of Science in Nursing from an accredited nursing program

This role offers compensation between $80 and $90 per completed evaluation, plus a mileage stipend and payment for administrative time.
