Enhance your clinical expertise and patient collaboration as a Nurse Practitioner conducting home health risk assessments throughout Columbus County, NC. This per diem position offers the flexibility to design your own schedule, working shifts in locations such as Tabor City, Chadbourn, Lake Waccamaw, and Whiteville to serve Medicare Advantage plan members in their homes. Each assessment takes around 45 minutes and visits may include annual health evaluations or post-discharge reviews, depending on the patients needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document prior and current medical history, conditions, and medications
- Educate patients on their diagnoses and prescription regimens
- Identify care gaps, providing recommendations to improve health outcomes
- No need to develop treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- CPR certification (BLS) currently active
- Board Certification from AANP or ANCC
- Minimum of six months recent NP experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
