Deliver essential home health risk assessments as a Nurse Practitioner, focusing on Medicare Advantage members throughout Marion County. This per diem position offers a high degree of flexibility, allowing you to tailor your schedule within the region to meet both professional and personal needs.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Opportunity to design your own schedule
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for eligible health plan participants
• Review and evaluate patients past and current medical and medication histories
• Educate patients on their health conditions and medications to promote understanding and self-management
• Identify and report gaps in care, offering recommendations for further action as needed
• Note: There is no requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Illinois
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• MSN from an accredited nursing program

For each completed evaluation, you will receive between $80 and $90, along with a mileage stipend and compensation for administrative tasks. The nature of the visits may include annual wellness checks or post-discharge assessments, with each visit averaging around 45 minutes.

Engage directly with individuals in their homes across areas such as Centralia, Salem, and Sandoval, supporting continuity of care and helping identify unmet health needs.
