Take on a meaningful role providing specialized care in a per diem position dedicated to home health risk assessments. Ideal for Nurse Practitioners who enjoy clinical independence, this opportunity allows you to support Medicare Advantage members throughout Oswego County with assessments designed to enhance health outcomes and continuity of care.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Choose your hours to meet a weekly or bi-weekly commitment

Primary Responsibilities:

• Perform thorough annual and post-discharge home health risk assessments for Medicare Advantage plan members.
• Travel to patient residences within Oswego County, including communities such as Fulton, Brewerton, Pulaski, and Central Square.
• Assess comprehensive medical, surgical, and medication histories.
• Deliver patient education concerning medical conditions and prescribed medications.
• Identify gaps in care and provide recommendations to promote optimal health.
• Evaluation visits do not require creating treatment plans or prescribing medications.

Qualifications:

• Active Nurse Practitioner license in New York State
• Board certification through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• Graduation from an accredited Master of Science in Nursing (MSN) program
• Minimum of six months of recent experience as a Nurse Practitioner preferred

Nurse Practitioners are compensated $80$90 for each evaluation completed, in addition to a mileage stipend and compensation for administrative time.
