Advance your clinical expertise in an impactful per diem role, conducting home health risk assessments for Medicare Advantage plan members across Oswego County. As a Nurse Practitioner, you will visit patients in their homesincluding areas like Fulton, Brewerton, and Pulaskito provide thorough annual assessments and offer educational support. This position allows you to tailor your work schedule, making it an excellent option for those seeking flexibility and autonomy.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment, based on preference
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, medical conditions, and medication lists
- Educate patients on their health conditions and prescribed medications
- Identify gaps in care and suggest appropriate measures to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York
- Current BLS (CPR) certification
- Board Certification from AANP or ANCC
- Minimum of 6 months' NP experience preferred
- MSN from an accredited nursing program
