Advance your clinical expertise while enjoying a flexible schedule as a Nurse Practitioner conducting home health risk assessments within Overton County, TN. Work directly with Medicare Advantage plan members across various locations, including Livingston, Hilham, and Alpine, delivering vital healthcare assessments that help improve patient outcomes in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments during in-home visits (approximately 45 minutes each)
- Interview patients to review prior/current medical and medication histories
- Assess and evaluate acute and chronic health conditions
- Provide education on health conditions, prevention, and medication adherence
- Identify gaps in care and recommend steps for resolution
- No medication prescription or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- MSN degree from an accredited program
- At least 6 months of nurse practitioner experience preferred
