Job Description

Make a meaningful impact by delivering home health risk assessments to Medicare Advantage plan members in Delta County. As an experienced Nurse Practitioner, youll provide essential assessments and education to patients residing in various communities, including Delta, Orchard City, and Paonia. This per diem opportunity offers maximum flexibilitycraft your ideal schedule with shifts available any day of the week.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingset your own hours
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct home health risk assessments (approx. 45 minutes) with visits ranging from annual evaluations to post-discharge assessments
  • Review prior and current medical history, evaluate conditions, and document medication history
  • Educate members about their chronic conditions and medication regimens, ensuring understanding and compliance
  • Identify gaps in care; provide actionable recommendations to improve health outcomes
  • No requirement for treatment planning or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Current BLS/CPR certification
  • Board certified through AANP or ANCC
  • Six months or more of recent experience as a nurse practitioner preferred
  • MSN degree from an accredited institution
