Enhance your impact as an experienced Nurse Practitioner by conducting home health risk assessments for Medicare Advantage plan members in Delta County, Colorado. This per diem, flexible-schedule opportunity allows you to shape your own workweek, providing a valuable service to patients residing in diverse areas across Delta County, including Delta, Orchard City, and Paonia.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform thorough home health risk assessments (average 45 minutes per evaluation) for Medicare Advantage members
- Evaluate patients medical history, current conditions, and medication use
- Educate patients about their health conditions and medications
- Identify and address care gaps, offering recommendations to enhance health outcomes
- Duties do not include treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Colorado
- MSN degree from an accredited nursing school
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of recent NP experience (preferred)
