Take your advanced clinical expertise directly into the homes of patients as a Nurse Practitioner specializing in home health assessments. This per diem opportunity offers flexibility and autonomy, allowing for impactful patient encounters in Cedar Hill and throughout Montgomery County, TN, including Woodlawn, Cunningham, and Sango.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly commitment to suit your availability
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members within the assigned areas
- Conduct in-depth evaluations of medical history, current health conditions, and medication use
- Deliver patient-centered education on chronic conditions and medications, promoting wellness and adherence
- Identify and document gaps in care; offer actionable recommendations for improved outcomes
- No requirement to prescribe medications or develop treatment plansfocus on assessment and education
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification from AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited program
- A minimum of 6 months of current nurse practitioner experience preferred
In this dynamic role, youll receive compensation ranging from $80 to $90 per evaluation, with additional mileage stipends and administrative time covered.
