As a Nurse Practitioner collaborating with Medicare Advantage plan members throughout Henderson County, TN, you will use your clinical expertise to enhance patient well-being and close gaps in care, all while enjoying the freedom to shape your work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Choose a minimum weekly or bi-weekly hourly commitment that suits your lifestyle
Key Responsibilities:
- Conduct comprehensive home health risk assessments for eligible Medicare Advantage plan members
- Perform thorough reviews of prior and current medical history, ongoing medical conditions, and medication use
- Offer clear education regarding disease management, medication regimens, and preventive health strategies
- Identify care gaps and recommend practical steps for improved outcomes
- Visits may include annual health assessments or post-discharge evaluations; each typically lasts 45 minutes
- No creation of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of six months Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
