Experienced Nurse Practitioners are needed for per diem home health risk assessments in Greene County, NY, serving Medicare Advantage plan members. This role offers flexibility, autonomy, and the chance to make a meaningful impact through comprehensive patient assessments in a home care setting.

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members in locations such as Catskill, Cozsackie, Ashland, Ravena, and surrounding areas
- Review prior and current medical history, evaluate medical conditions, and assess medication usage
- Educate patients on their medical conditions and medication regimens
- Identify gaps in care and provide recommendations to support optimal health outcomes
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York State
- MSN degree from an accredited school of nursing
- Board Certification through either AANP or ANCC
- Current CPR (BLS) certification
- A minimum of 6 months of recent NP experience preferred
