In this per diem opportunity, youll travel throughout Barry County, conducting in-home health risk assessments for Medicare Advantage members. Locations may include Monett and surrounding communities such as Savannah, Country Club, and Fillmore. Each evaluation typically lasts about 45 minutes and may involve an annual health review or a post-discharge assessment.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Build your own schedule
- Commit to a minimum number of hours weekly or bi-weekly
Core Responsibilities:
- Perform detailed annual risk assessments in patients homes, focusing on thorough evaluation of current and past medical history, chronic conditions, and medication regimens
- Provide clear patient education related to medications and health conditions, empowering individuals to participate in their care
- Identify potential care gaps and offer documented recommendations for improvement
- No requirement to prescribe medications or develop ongoing treatment plans
Desired Qualifications:
- Valid Nurse Practitioner license issued in Missouri
- Master of Science in Nursing (MSN) from an accredited institution
- Board certified through AANP or ANCC
- Current BLS/CPR certification
- At least 6 months of nurse practitioner experience preferred
- Strong assessment, documentation, and interpersonal skills
