Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members in various home settings
• Review and evaluate patients medical history, current health conditions, and medication regimens
• Offer clear, understandable education about health conditions and prescribed medications
• Identify gaps in patient care and recommend appropriate actions to address these gaps
• No requirements for designing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• Minimum of 6 months recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
