Deliver high-quality patient care and play a crucial role in preventative health for Medicare Advantage members in Wake County. As a Nurse Practitioner specializing in home health, you will perform targeted risk assessments in patient residences throughout areas such as Cary, Raleigh, and Apex. This per diem opportunity offers full flexibility, allowing you to tailor your schedule while providing an essential service.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Option for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes
- Review prior and current medical history, health conditions, and medications
- Educate patients on chronic conditions, preventive care, and safe medication practices
- Identify care gaps and supply recommendations to promote quality outcomes
- Focus strictly on assessmentsno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- Minimum six months of NP experience preferred
This role offers a competitive pay structure, with $80-$90 per completed evaluation. Mileage reimbursement and compensation for administrative time are also provided.
