Exciting opportunity for a dedicated and skilled Nurse Practitioner interested in providing vital home health risk assessments. This per diem role involves traveling to Medicare Advantage plan members homes in Durham County, NCincluding areas such as Raleigh, Morrisville, and Cary. The flexibility of this position allows you to tailor your work schedule and make a meaningful impact in your community.
Schedule:
- Flexible shifts available between Monday and Sunday
- Option to create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in their residences
- Assess patients medical history, current conditions, and medication use
- Deliver education regarding medical conditions and medications
- Identify care gaps and provide recommendations for follow-up
- No prescribing or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- Minimum of six months experience as a Nurse Practitioner preferred
