Take on a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you will visit Medicare Advantage plan members in their homes across Wake County, offering crucial assessments that support ongoing patient care and health outcomes. Locations may include Raleigh, Cary, Apex, and surrounding areas, with assignments varying by shift.

Enjoy true autonomy by setting your own flexible scheduleshifts are available seven days a week, and you choose your hours with only a minimum weekly or bi-weekly commitment required. This position lets you balance professional growth with your personal life.

Key Responsibilities:

  • Complete annual home health risk assessments on Medicare Advantage plan members in their residences
  • Document and evaluate patients current and prior medical conditions, medical history, and medication regimens
  • Provide clear education to patients about their conditions and prescribed medications
  • Identify gaps in care and make recommendations to address them
  • Assignments may include annual health assessments or post-discharge visits
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited school of nursing
  • At least 6 months of current NP experience preferred

This position offers $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
