Experienced Nurse Practitioners are invited to take on a vital role conducting home health risk assessments for Medicare Advantage plan members throughout Durham County, with flexible, per diem opportunities. Youll be visiting patients in their residences, providing essential wellness evaluations and bridging the gaps in careall without the requirements of prescribing medications or creating treatment plans.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to meet weekly or bi-weekly commitments
Key Responsibilities:
- Complete comprehensive, annual home health risk assessmentsmost visits average 45 minutes per evaluation
- Assess patients medical, surgical, medication, and family histories, focusing on current and prior conditions
- Deliver education to patients regarding their medical conditions and proper medication management
- Pinpoint gaps in care and suggest actionable recommendations for resolution
- Visits may be annual assessments or post-discharge check-ins
Qualifications:
- Valid Nurse Practitioner License in North Carolina
- Board Certification from AANP or ANCC
- Current BLS (CPR) certification
- MSN degree conferred by an accredited school of nursing
- At least 6 months of nurse practitioner experience preferred
In appreciation for your expertise, each evaluation is compensated between $80 and $90, with additional stipends for mileage and administrative time.
