Advance patient care by delivering in-home health risk assessments to Medicare Advantage members throughout various communities in Johnston County, NC. This per diem Nurse Practitioner role offers autonomy, flexibility, and a dynamic patient-centered environment. Tailor your work schedule to your needs, choosing flexible shifts between Monday and Sunday while making a meaningful impact on members health and well-being.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling allows you to create your own workweeks
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical history, current conditions, and medication regimens
- Educate patients about their health conditions and prescribed medications
- Identify gaps in care, providing consultative recommendations to help close those gaps
- Focus exclusively on assessment and educationno treatment plans or prescriptions required
Qualifications:
- Unrestricted Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of NP experience preferred
- MSN from an accredited nursing program
