Job Description

Advance patient care by delivering in-home health risk assessments to Medicare Advantage members throughout various communities in Johnston County, NC. This per diem Nurse Practitioner role offers autonomy, flexibility, and a dynamic patient-centered environment. Tailor your work schedule to your needs, choosing flexible shifts between Monday and Sunday while making a meaningful impact on members health and well-being.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-scheduling allows you to create your own workweeks
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and evaluate patients medical history, current conditions, and medication regimens
  • Educate patients about their health conditions and prescribed medications
  • Identify gaps in care, providing consultative recommendations to help close those gaps
  • Focus exclusively on assessment and educationno treatment plans or prescriptions required

Qualifications:

  • Unrestricted Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of NP experience preferred
  • MSN from an accredited nursing program
