This per diem position provides autonomy in your scheduling and the chance to make a significant impact through comprehensive patient evaluation in home environments.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Evaluate patient history, current medical status, and medication use
• Deliver education on medical conditions and medications to patients and their families
• Identify care gaps and make recommendations for resolution
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in NY
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• At least 6 months of recent NP experience preferred
• MSN from an accredited nursing program

In this role, youll travel to various communities in Jefferson County, including Le Ray, Watertown, Adams, and Fort Drum, conducting in-home visits that typically last about 45 minutes. Youll play a pivotal part in ensuring high-quality preventive care while enjoying autonomy and a supportive scheduling structure. In addition to an evaluation-based pay of $80-$90 per visit, youll receive a mileage stipend and compensation for administrative time.
