Make a meaningful impact on patient wellness by providing expert home health risk assessments for Medicare Advantage plan members across Jefferson County, NY. This per diem opportunity offers nurse practitioners the flexibility to create their own schedules, working shifts throughout the weekincluding evenings and weekends. You will deliver important health evaluations in environments such as Le Ray, Watertown, Adams, Fort Drum, and surrounding locations, empowering patients through education and tailored recommendations.

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Perform thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior/current medical and medication histories
- Educate members about their medical conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations
- Visits average 45 minutes; may include annual assessments or post-discharge evaluations
- No development of treatment plans or medication prescriptions required
Qualifications
- Valid Nurse Practitioner license in New York State
- Current CPR certification (BLS)
- Board Certification: AANP or ANCC
- At least 6 months of nurse practitioner experience preferred
- MSN degree from an accredited nursing program
