Step into a meaningful role serving Medicare Advantage plan members by providing in-home health risk assessments throughout Jefferson County, NY. This per diem opportunity is designed for Nurse Practitioners with a passion for patient-centered care and flexibility in their work schedule. Visits typically last about 45 minutes and may include annual health assessments or post-discharge visits in areas such as Le Ray, Watertown, Adams, and Fort Drum.
Schedule:
- Flexible shifts available any day of the week (Monday through Sunday)
- Self-schedulingcreate your own shifts
- Minimum weekly or bi-weekly hourly commitments to fit your availability
Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage members
- Review medical history, current conditions, and medication lists
- Educate members about their health status and medications
- Identify gaps in care and suggest interventions
- Focus solely on assessmentno development of treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York
- Current BLS/CPR certification
- Board-certified by AANP or ANCC
- Completion of an MSN degree from an accredited program
- Minimum of 6 months of recent experience as a Nurse Practitioner is preferred
This position offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
