A skilled Nurse Practitioner is needed for flexible per diem work conducting home health risk assessments within Saline County, MO. The role involves visiting Medicare Advantage plan members in their homes, with visits averaging approximately 45 minutes. Locations include Marshall, Slater, Sweet Springs, and surrounding areas. Shifts are self-scheduled, allowing you to select your availability, with a minimum weekly or bi-weekly commitment.
Schedule:
- Flexible shifts available seven days a week (MondaySunday)
- Build your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, chronic conditions, and medication usage
- Educate patients regarding medical conditions, treatment, and prescribed medication
- Identify care gaps and recommend steps for follow-up or improvement
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Missouri
- MSN from an accredited nursing program
- Current BLS/CPR certification
- Board Certification via AANP or ANCC
- At least six months of recent experience as a Nurse Practitioner preferred
